The CCG is looking to appoint a knowledgeable, enthusiastic and motivated professional to join our Transforming Care Programme team at NHS Kernow as a clinical reviewer.

This post supports the Learning Disability Programme Manager with the Transforming Care Programme (TCP) to carry out Care and Treatment Reviews (CTRs) and Blue Light meetings and to assess, review and commission robust, safe and appropriate care packages for adults with a learning disability for NHS Kernow.

To facilitate the discharge of individuals with learning disabilities and/or autism from out of area hospitals back to their local communities, in line with the principles of Transforming Care and Section 117 (Mental Health Act) protocol.

You need to have knowledge and experience of working with adults with complex mental health and learning disability needs, excellent communication and organisational skills and be able to work with a high degree of autonomy.   The CCG is committed to personal development and you will be supported to deliver the role.

This is a full time post working 37.5 hours per week and this will need to be done on a flexible basis to meet the needs of the families and service. There will be extensive travel across the County and out of county. There is some degree of unsociable hours working within the week.

Secondment may be considered.
